for the fiscal year ending June 30, 2013:

RESOLUTION 12-313B ACCEPT RESIGNATION SHERIFFS DEPARTMENT
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby accepts the resignation of Preston
Gebhart, Sheriff's Department effective 12-15-12 and any accrued benefits to be paid out and removed from payroll.

RESOLUTION 12-313C HIRE SUBSTITUTE COOK NUTRmON-MARTENSDALE
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves the hiring of Bonnie Groth,
substitute cook for the Nutrition Department at the Martensdale Meal Site effective December 18, 2012 at an houdy
rate of $8.78 with no eligible benefits. Carried unanimously
RESOLUTION 12-313D INCREASE OF HOURS IN NUTRITION DEPARTMENT
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves to increase the hours worked
for Susan Keeney, cook from 5 hours to 6 Yz hours. She is at a regular part-time status and is eligible for benefits.

RESOLUTION 12-313E SUSPENSION OF TAXES
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby directs the County Treasurer to suspend
the 2 " half of 2011 taxes on the following property as provided in Iowa Code 427.9:

BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves the taxes on the mobile home

RESOLUTION 12-313G APPROVE WARREN COUNTY GOVERNMENT CONTINUITY OF OPERATIONS PLAN
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves Warren County Govemmen
Continuity of Operations Plan. Carried unanimously
RESOLUTION 12-314 ACCEPT RETIREMENT SHERIFF'S DEPARTMENT
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves to accept the
retirement/resignation of Bob Gebhart in the Sheriff's Office after 28 years of employment, effective December 31,
2012, and to pay any accrued vacation, comp time, and any sick leave benefits entitled upon retirement, and to
remove from payroll. Carried unanimously
RESOLUTION 12-315 ACCEPT RETIREMENT SHERIFF'S DEPARTMENT
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves to accept the
retirement/resignation of Nell Gurwell in the Sheriff's Office after 21 V years of employment, effective December 31,

RESOLUTION 12-316 IOWA DOT ADDENDUM 1-35 PROJECT
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves to accept the Iowa Department
of Transportation Addendum to Agreement No. 2012-c-062 regarding 1-35 Project. Carried unanimously
RESOLUTION 12-317 ACCEPT AND APPROVE QUOTES FOR 168 TM AVE BRIDGE REPLACEMENT PROJECT
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby move to accept quotations from
Calhoun-Bums And Associates to provide final bridge design services for the 168 = Avenue bridge replacement
project in the amount of $55,940.00. Unanimously carried
RESOLUTION 12-318 APPROVE PURCHASE OF EQUIPMENT
BE iT RESOLVED THAT, the Warren County Board of Supervisors hereby move to approve the purchase and
installation of ProMac horizontal flail brush cutting head onto County's 318 Cat Track Hoe from Ziegler Equipment at
a cost of $41,745.00. Carried unanimously
RESOLUTION 12-319 REVIEW & ACCEPT QUOTATIONS FOR NEW MOTOR GRADER
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby move to accept the following informal
quotations from the following vendors:
Murphy Tractor & Equipment $207,102.00
Scott Van Keppel $219,566.00
Ziegler Cat $225,922.00
And, approve the quotation from Murphy Tractor & Equipment in the amount of $207,102.00 to purchase one new
motor grader for the County. Carried unanimously
RESOLUTION 12-320 APPROVE TO SIGN CONTRACT
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby move to approve the Chair to sign a
contract for wetland monitoring services with Alex Davis of Ames, Iowa at a rate of $12.50. Car;'ied unanimously
RESOLUTION 12-321 OPEN PUBLIC HEARING
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby enters into the public hearing portion of
the meeting. Carried unanimously
RESOLUTION 12-322 CLOSE PUBLIC HEARING
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves to close the public portion of
the meeting. Carried unanimously
RESOLUTION 12-323 REZONING
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves to rezone 3.3 acres, Parcel G,
7328 Hwy 92, Indianola, Iowa belonging to the Leonard L. and Beverly J. Peek Trust. The said property will be
rezoned from A-1 to C-2. Carried unanimously
RESOLUTION 12-324 OPEN PUBLIC HEARING
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby enters into the public hearing portion of

BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves to close the public portion of
the meeting. Carried unanimously
RESOLUTION 12-326 APPROVE REZONING REQUEST
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves the rezoning request for Mark
and Janna Putz, 17028 Hwy 92, Indianola, IA from A-1 to C2R with the following restrictions thereto:
1 .) Ownership to remain in the name of Mark and Janna Putz.
2.) The zoning shall revert to A-1 if the business is moved or the property is sold,
3.) Business shall only employ the Putz family.
4.) Any signage other than that affixed to the building where the business is conducted shall not
exceed 2X2 feet square and shall not be neon.

RESOLUTION 12-327 MENTAL HEALTH/EYERLY BALL
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves the Eyerly Ball Contract with
one revision being that the Patient Assistance application fee has been adjusted from $20.00 to $20.52. Carded

RESOLUTION 12-328 CONVENE CLOSED SESSION
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves to convene closed session
according to Iowa Code 21.5 (1)(a). "To review or discuss records which are required or authorized by state or
federal law to be kept confidential or to be kept confidential as a condition for that governmental body's possession
or continued receipt of federal funds."

Calhoun-Bums And Associates to provide final bridge design services for the 168 = Avenue bridge replacement

RESOLUTION 12-318 APPROVE PURCHASE OF EQUIPMENT

BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby approves the rezoning request for Mark
and Janna Putz, 17028 Hwy 92, Indianola, IA from A-1 to C2R with the following restrictions thereto:
1 .) Ownership to remain in the name of Mark and Janna Putz.
2.) The zoning shall revert to A-1 if the business is moved or the property is sold,
3.) Business shall only employ the Putz family.
4.) Any signage other than that affixed to the building where the business is conducted shall not
exceed 2X2 feet square and shall not be neon.
