RESOLUTION 11-31
GRANT & PERMIT FORM
WHEREAS, as provided in Section 319.14 of the Code of Iowa, whereby a per-
son shall not excavate or fill or make any physical change within the right of way
of public road or highway without obtaining a permit from the highway authority
having jurisdiction of such public road or highway, and any work performed un-
der the permit shall be performed in conformity with the specifications prescribed
by the highway authority; and, WHEREAS, Warren County has adopted a "Grant
and Permit" form, including specifications for performance of the said work, in-
cluding the filing of a cash bond to insure compliance with the said specifica-
tions; now,
BE IT RESOLVED,
1. that the amount of each bond to be posted with the "Grant and Permit" appli-
cation be established as follows:
Trenching the full width of a paved road $1000 cash bond
Trenching half the width of a paved road $500 cash bond
Boring under a paved road $250 cash bond
Trenching on an unpaved road $1.50 per lineal foot
Note: A cashier's check may be submitted in lieu of cash for any private utility
placed in highway or street right of way.
2. that the applicant shall be the owner of the proposed improvement;
3. that the applicant is responsible for determining the location of any under-
ground utilities and the protection of all nearby utilities• Carried Unanimously.
RESOLUTION 11-32
CERTIFICATION OF FARM TO MARKET PROJECTS
WHEREAS, under the provisions of Chapter 310.19 of the Code of Iowa, whereby
the
County Engineer is charged with the duty of supervision, inspection and direc-
tion of the construction of Farm-to-Market projects; now,
BE IT RESOLVED THAT, the County Engineer of Warren County, Iowa, be and
is hereby designated, authorized and empowered on behalf of the Board of Su-
pervisors of Warren County to execute the certification of completion of work
and final acceptance thereof in accordance with plans and specifications there-
fore in conjunction with all Farm-to-Market construction projects in this county.

RESOLUTION 11- 33
APPROVE 28E AGREEMENT
WHEREAS, Des Moines Area Metropolitan Planning Organization has submit-
ted a 28E Agreement with Warren County, now;
BE IT RESOLVED THAT, the Warren County Board of Supervisors move to ap-
prove the 28E Agreement between Warren County and Des Moines Area Metro-
politan Planning Organization.
Carried Unanimously.
RESOLUTION 11-34
EQUIPMENT PURCHASE
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby move
to approve the purchase of a 2011 Super Duty Chassis Cab Truck from Noble
Ford in the amount of $26,936.00 and the purchase of a Omaha Box for the truck
from Hawkeye Truck Equipment in the amount of $14,698.00 for a total of
$41,634.00. Carried Unanimously.
RESOLUTION 11-35
ROAD VACATION-245 TM AVENUE & JESUP STREET
WHEREAS, pursuant to the provisions of Chapter 306 of the Code of Iowa,
whereby the Board of Supervisors may vacate and close a road in the interest of
the public;
See exhibit "C" & Exhibit "D";
WHEREAS, proponents and opponents of the proposed closure were given the
opportunity to be heard; and,
WHEREAS, the Chairman asked for any written objections to or support of, the
said closure; now, BE IT RESOLVED THAT, the above described section of the
road be officially vacated and closed and BE IT RESOLVED by Warren County
that the said part of the present right of way for 245 th Avenue and for Jesup
Street herein described in Exhibit "C" and "D" together with any and all rights of
Warren County in and to the same are hereby abandoned, and Warren County
hereby disclaims any right, title or interest in and to said part of present right of
way or to the use thereof for any purpose whatsoever, with any existing utilities
retaining the right to remain in abandoned road right of way, as per Iowa Code
Section 306, provided that said resolution shall be effective on the date that the
Agreement Covering Removal of Grade Separation Structure and Relocation of
245' h Avenue, DOT 605 706 W M.P. 51 97 Trenton Subdivision, Beech, Iowa is
executed by and between Union Pacific Railroad Company and the Warren
County, Iowa Board of Supervisors, said document to be executed no later than
March 14, 2011. Carried Unanimously.
RESOLUTION 11-36
APPROVE MH/DD EXCEPTION TO POLICY
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby ap-
proves the Exception to Policy with the payment cap being $457.00 per day for
no longer than 3 months.

RESOLUTION 11-37
APPROVE MICRO-DISTILLED SPIRITS PERMIT FOR BROADBENT DIS-
TILLERY
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby ap-
proves a Micro-distilled Spirits Permit for Broadbent Distillery. Carried Unani-
mously.
RESOLUTION 11-38
APPROVE CODE OF ORDINANCE AMENDMENTS
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby ap-
proves the amendments to Chapters 1,3, & 15 in the Code of Ordinances. See
Code of Ordinance in Auditor's Office for amendments. Carried Unanimously.

APPROVE COMMUNICABLE AND INFECTIOUS DISEASES ORDINANCE
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby moves
to approve the Communicable and Infectious Diseases Ordinance Chapter 37 in
the Code of Ordinances.

APPROVE LEASE AGREEMENT
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby moves
to approve the Lease Agreement between Warren County and South Central
Youth and Family Services. Carried Unanimously.
