9:30 a.m. with Supervisors' Doug Shull, Dean C. Yordi, and Supervisor Crystal

RESOLUTION 16-22A WAGE INCREASE AUDITOR'S OFFICE
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby ap-
proves a change in wages for Sharon Wiley, in the Auditor's office, effective
January 1, 2016 resulting in an increase to $20.31 per hour due to additional
duties and responsibilities. Sharon's salary will move from General Supplemen-
tal/elections to General Fund as function 9010. Carried unanimously

BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby ap-

Bridge Replacement for project No. BROS-C091 (102)--5F-91 and authonze the

RESOLUTION 16-25 ACCEPT AND APPROVE BIDS FOR 65TM AVE
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby ap-
proves the County Engineer's review of bids from Contractors for Project No.
LFM-M2453~7X-91, and accept the bids from:
Nelson & Rock Contracting $157,054.00
Herberger Construction $197,135.00
Jensen Construction Co. $211,645.00
And approve the bid from Nelson & Rock Contracting in the amount of
$157,054.00 and authonze the Chair to sign the contract. Carried unanimously
RESOLUTION 16-26 APPROVE CONTRACT 30TM AVENUE BRIDGE RE-
PLACEMENT
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby ap-
proves the Contract with Savage Dirt Works, LLC for the 30t" Avenue Bndge
Replacement for project No. L-BrN0413---73-91 and authorize the Chair tosign.

RESOLUTION 16-27 APPROVE PROPOSAL SHIVE-HATFERY SECONDARY
ROADS
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby ap-
proves the proposal with Shive-Hattery, Inc. for the Secondary Roads Facility
Master Planning. Carried unanimously
RESOLUTION 16-26 DUST CONTROL
WHEREAS, numerous requests are received by Warren County for the place-
ment of dust control on various sections of local secondary roads, now;
BE IT RESOLVED on a motion by Dean C.Yordi, second by Crystal Mclntyre,
that the following policy be adopted for the placing of dust control matedal on the
local secondary road system commencing in the 2016 calendar year:
Dust control material placed on any local secondary road shall be at the re-
questing party's expense.
1. No dust control material is to be applied on any local secondary read
except by vendor(s) approved by the Warren County Engineer.
2. Information regarding the application of dust control matenal on any local
secondary read shall be obtained from the vendor.
3 The type of dust control ~a, terial ~o 12e used must .be app,reved by the
Warren County Engineer,prier te~ts,applcatiea o~,any k~:~l se¢or,.~i~Fro~Ba
4. Any potholes, washouts, secondary ditches, etc., which may develop in
the treated surface may result in Warren County scanfying and blading the sur-
face without notice.
5. The requesting party shall be responsible for the placement and mainte-
nance of signs and flags designating the outer limits where the dust control
matedal is placed. Failure to do so may result in Warren County bleding the
involved area, for which Warren County assumes no liability.
6. Warren County will not necessanly place rock on dust control locations.
7. Requesting parties applying for dust control must sign and date a form
provided by the vendor acknowledging they have received a copy of this dust
control policy. This completed form shall be submitted by the vendor(s) to the
Warren County Engineer prior to the placement of dust control materials.
8. Warren County reserves the right to perform any maintenance operations
within the treated surface area and the adjacent right of way.

RESOLUTION 16-30 CENTRAL IOWA COMMUNITY SERVICES MENTAL
HEALTH REGION
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby ap-
proves becoming an employing county for Central Iowa Community Services
Mental Health RegiOn. Carried unanimously
RESOLUTION 16-31 STATEMENT OF UNDERSTANDING CENTRAL IA COM-
MUNITY SERVICES
BE IT RESOLVED THAT, the Warren County Board of Supervisors hereby ap-
proves and authorizes Chair to sign Statement of Understanding with Central
Iowa Community Services regarding employment of Betsy Stursma and Marci
Kellow. Carried unanimously
